At your home for healthcare, you will not be turned away based on your ability to pay. We offer general services to uninsured and underinsured adults including HIV/STD testing, TB Screenings, On-site lab services, specimen collection, On-site pharmacy with prescription medication for eligible patients, and vision screening.


A Federally Qualified Health Center (FQHC) is a “safety net provider” which provides essential medical services to the most disadvantaged and vulnerable populations. FQHCs must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.

You can schedule an appointment three ways:

  1. Sending an appointment request on the website
  2. Call any of our locations and speak with one the intake specialists.
  3. Stop by any of our locations, and schedule an appointment in person.

NMC has walk-in hours between 8:00 am -11:00 am and 2:00 pm- 4:00 pm, for medical and dental. Please be advised you may experience a longer than usual time because walk-in appointments are worked in with the normal schedule.

Due to the time it takes to complete a physical, it cannot be completed as a walk-in appointment. Walk-in appointments are for acute care only.

You will need your insurance cards, photo identification, and your-copayment. If you do not have insurance you can bring in proof of the following to qualify for the sliding scale discount: residency, identification, and income.

When you schedule your visit, please plan on arriving 15 minutes prior to your scheduled appointment time.

For your convenience, our practices accept cash, money orders, debit and/or credit cards (Visa, MasterCard, Discover, and American Express). We will charge your credit card only for the amount you authorize.

Due to the Health Information Portability and Accountability Act (HIPAA) we cannot disclose patient information to anyone, regardless of relationship, other than the patient without their consent. You may fill out a release of information form that will allow the doctor’s office to speak to the person you designate. Please see our Patient Privacy Notice for more information.

For your comfort, it would be best to bring a relative or representation with to your appointment. We do have staff members who speak the following languages to assist if the need arises: Spanish, Creole, and German. We can also contact a sign language interpreter on the behalf on the patient as well.

No, NMC does have physicians who do DOT physicals.

Yes, on the first Thursday of every month at the Richardson-Lewis Center we are open until 8:00 pm, taking appointments until 7:00 pm. On the third Tuesday of every month at the Lincoln Center, we are open until 8:00 pm, taking appointments until 7:00 pm.

Yes. On the first and third Saturdays at the Smith-Williams Center patients can schedule a medical appointment; likewise for dental appointments at the Cecil V. Butler Center in Havana. We are open until 2:00 pm, taking appointments until 1:00 pm.

Yes-any available provider may see you. Our Providers work as a team and your records are available to all our Physicians. We also have a doctor on call that will have your clinic chart with your medical history available for assistance.

That depends on what labs your doctor ordered. Some results, like cholesterol and lipids come back fairly quickly. Others, like some chemistry tests and pathology reports can take up to two to three weeks. Still others, such as culture growths may need to get sent back for further testing based on the results of the initial test. X-Ray and ultrasound reports are typically returned within a week.

You may complete a records request at our office, print a release from the “Printable Documents” portion of the website, or if you are seeing a physician elsewhere they can request the records on your behalf. Make sure you have the name, address, phone and fax number of the place you want records sent to/from. Requests can be faxed to our Medical Records department at 850-487-9915.

There is no charge if a physician requests your records or if you are requesting to be sent to another physician. Otherwise, the charge for medical and dental records is $1 per page for the first 25 pages, then $0.25 per page for any additional pages. Any request for images will be $3.00 per disc.

NMC policy is seven to ten business days after we receive the initial request, and if applicable payment. Under certain special circumstances that time can be shortened.

Yes, all of our practices use the patient portal from our practice management system, AthenaHealth. In order to sign up, provide your email address at register to receive a link via email or ask for intake specialist to provide you with the website link. Please note, not all medical information will be sent through the patient portal.

Neighborhood Medical Center provides access to mental health services to all of our patients, but it is our policy that the patient has both Primary Care and Mental Health at Neighborhood Medical Center as a way to effectively manage the patient’s care. Each patient should begin with Primary Care and if mental health services are needed then a referral will be made by the primary care provider.

No, NMC provides services to all patients regardless of their insurance coverage and the patient’s ability to pay.

The sliding fee program allows us to reduce or “slide” the fees for the care of you or your family at NMC. You can apply for the program if you need assistance to help you pay for your care. The slide consists of categories with a set co-pay to be paid at the time of visit and discounted percentage from the total remind of the balance. For example, Slide B co-pay is $25.00 and you will receive 75% off the reminder of the medical services.

Complete an application. Eligibility is based on gross income and family size. You can bring in proof of residency, proof of income, and proof of identification; based upon that information the slide will be calculated. The co-pay range is from $5 to $70 which is due to prior to services render.

The following are acceptable proofs of residency:

  • Valid and unexpired driver’s license or state-issued ID card (with Leon County residential address for Care Net)
  • A lease or shelter statement from the applicant’s landlord which contains the current address and lease date(s)
  • Current utility bill in applicant’s name and current address
  • Mortgage payment book and/or tax receipt
  • Homestead exemption document
  • Vehicle registration showing current address.
  • Voter’s registration card with current county residential address (no absentee)
  • Employment record
  • Documentation of residency documented through County, State, or Federal agency
  • Statement of Residency from homeless shelter or recognized community based program.
  • Notarized statement of Residency from who the patients resides with, AND the person writing the letter must provide their residency

The following are acceptable proofs of income:

  • Paycheck stubs
  • Recent Income Tax Form (past year)
  • Employment Verification (must be on letterhead)
  • Rental Income
  • Quarterly Tax Report
  • Food Stamps (only for Care Net)
  • Alimony
  • Unemployment Compensation
  • Social Security Benefits (SSI )

  • Bank statement (must show direct deposit and from what employer)

  • Business Checks for payment of wages or salaries

The following are acceptable proofs of identification:

  • State Driver’s License

Every year, unless some information has expired and/or changed that needs to be updated before the year date on the application.

Your co-pay is $70, and no discount is applied.

NMC offers payment plans, where you can pay a minimum balance every month.

Your copay and deductible and any non-contracted usual and customary fees. If you do not have insurance you are expected to pay in full at time of service. If you have qualify for the sliding scale discounts, the nominal fee and/or co-pay designated by the scale.
